All of our physicians have a full-time post hospital medical practice based on-site in various post-acute facilities where our services are provided. It is in these facilities where we specifically provide geriatric, physical medicine and rehabilitation, and general internal medical services to patients in various types of post-acute and other long-term care settings.

The intensity of services will vary with patient need and medical necessity. Patient acuity is increasing dramatically, and as such, physicians need to have a medical practice that fits into a true interdisciplinary environment and is specialized in the care of patients in post-acute facilities.
